At this time, Dr. Smith is an out-of-network provider for all insurance carriers. You will be given an receipt for the services provided to submit to your insurance company with all necessary information.
Insurance that includes out-of-network benefits often reimburses 20-90% of fees.

CANCELLATION POLICY
Dr. Smith asks that patients provide 24 business hours notice prior to cancelling any appointment. If patients do not cancel in this time frame or fail to come to an appointment, they will be charged the full fee. This excludes any medical emergencies.
